Well the first problem is your using 1&1 for webhosting... (Ok Ok that's my personal opinion but still).

The settings look correct, however your root domain will automatically load your www folder so why would you append /www/index.php? You should just type domain.com/index.php.

If this is loading the placeholder page, you can always create a test.html in your www directory (just put in dummy text) and then access it - domain.com/test.html. If it loads then it something with 1and1 and you'll need to contact them.
