Solution

I store image names in the database. For some reason in the database some of the photos were with uppercase extension and some of them were with lowercase.

E.G.

<img src="myimage.jpg"> Failed to render
<img src="myimage.JPG"> No problem Rendering

So be careful with that. Cheers.
