Solution

So basically at the moment you have a one way friendship, or a follow system similar to Twitter. If you want to make it a two way relationship you might consider an approval process similar to Facebook. Then simply use the same query but in reverse.

OK that sound like a good solution, I was always thinking in Laravel way (with elequent) and not "out of the box" :)
