Solution

i haven't used this form builder thingy. if it's creating html5 inputs you can try replacing "month" with "date"

That did to the trick - I haven't tried inputting into the database underneath my date column, as I'm busy with other work at the moment, but for the time being, this is the answer I was looking for.
